This paper examines the impact of acquired brain injury (ABI) on cognitive-communication disorders (CCD) and their widespread effects on social, family, educational, and vocational participation, emphasizing the need for flexible and individualised rehabilitation approaches due to the complex and chronic nature of these disorders. It highlights recent research on innovative, person-centred interventions—including telehealth, co-designed programs, and the Social Brain Toolkit—that enhance accessibility, engagement, and real-world communication outcomes for people with ABI. The paper also addresses the translation of evidence-based research into clinical practice, outlining frameworks, guidelines, and digital resources that support clinicians in delivering effective, individualized care while advancing knowledge in cognitive-communication rehabilitation.
